Things to bring to your first appointment:
1) A picture ID (see the “Red Flag Rules” preventing identity theft, to find out why).
2) Your insurance card, if you have one, and/or cash or check. See fees and payment plans for details.
3) Completed registration and medical history form, or your child’s medical history form – or arrive 30 minutes early to fill these out, please. Note: if you’re unable to complete these forms before your visit, we may not be able to complete your visit in the time allotted.
4) ALL your medications, remedies, and supplements – even the empty ones (empty bottles).
5) Please read the policies and privacy documents, so you know what to expect.

I’d be happy to meet with you/your family for a free consultation, just to talk, and see if we want to work together. Just schedule a 30-minute appointment.
When asked the purpose of the visit, just write “free consultation.”
Please be aware that, legally, I cannot give medical advice or treatment during this visit.

INSTRUCTIONS
1. Click on “Click here to make an appointment” above, then on “Make appointment for clinic.”
Right under the calendar, choose a “30 min” OR a “1 hour” appointment. (Please select 1 hr for a first visit.)
2. Create an account with your contact information, and to answer a couple of questions. These help me open your chart and prepare for your visit.
3. Choose to receive:
— “notification,” telling you the appointment was successfully entered into the program, but not yet confirmed by the doctor;
— “confirmation”, telling you that the doctor confirmed your appointment;
— and/or “reminder”, sent to you as your visit draws near.
After this, click “Make Appointment.”
4. Near the bottom of the next screen, your new user name and password appear.
Change your password to something easier to remember.
In the “Change username/password” screen, just erase what’s already in the boxes and fill them in with something you can remember.
Not your first time?
1. Click on “Click here to make an appointment!” above. A new window will open.
2. Click the “My Profile” tab at the top of the home page. This will let you log in.
3. After you log in, you’ll always have the option to change your username and password (to something easy to remember), under the tab marked “My Profile.”
4. Forgot your password? Email me, and I’ll send it to you.
5. Need to be seen today/tomorrow? Please call my cell phone, and I can manually book your appointment, if I have an opening.
